1. Look at the first three ingredients

What you should look for: If you are buying a product that claims to be whole wheat or grain, ensure that the first ingredient listed is “whole wheat” or “whole grain” and that sugar is not in the top few. Ingredients are listed in order of predominance. Highly processed white flours go under the alias of ‘enriched’ and ‘wheat’ flour. They are no better than their bleached brothers. Look for the “Whole Grains Stamp” (http://www. wholegrainscouncil. org) on products for an easy way to find the best in whole grain foods.

What you should know: Foods that tout being ‘wheat’ or ‘grain’ often contain more processed ingredients than not. For example, Kashi 7-grain crackers contain more ‘wheat’ (white) flour than whole grain flour.

What can it do: A study published by the U. S. Department of Health found if one group of people ate 1,200 calories worth of sugar and refined product, and another ate the same amount of calories worth of fruits, vegetables and whole grains, group #1 would gain weight and have a higher threat of disease.
Group #2 would not.

2. Scan for fat sources: Hydrogenated Oils

What is it? Hydrogenated oils are essential fatty acids that are chemically converted into a more stable source of fat. By forcing hydrogen into oil at high pressure, the original unsaturated fats are converted into trans fats, which have been linked to CVD.

What can they do? When hydrogenated oils are digested, they cause inflammation inside of the body, signaling the deposition of cholesterol to artery walls. That means clogged arteries heart disease. What are they in? Hydrogenated oils, used as an inexpensive source of fat, can be found in many cookies, crackers, and breads.

How can I avoid them? Look at the ingredients. Ensure that there are no trans fats, hydrogenated or partially hydrogenated oils listed. Note that companies are allowed to label a product as “trans-fat free” despite some trace (1%) in the ingredient list. Even this small amount can be harmful. Choose fats from sources such as: olive oil, canola oil, trans fat-free margarines, nuts and seeds.

3. Locate sugar source:

When the liver is unable to process fructose, it turns it into fats that are harmful for your heart and arteries. Fructose, unlike other sugars does not stimulate insulin, leptin or ghrelin production - all of which help indicate to your brain that you are full. High Fructose Corn Syrup (HFCS) is a modified fructose, made to be sweeter and cheaper, and because of this, is present in a majority of packaged foods. As a general rule, it’s best to avoid these types of sugars.

How much sugar is too much? A good rule of thumb is the higher in sugar and carbohydrates and lower in fiber, the worse it is.

The National Organization of Circumcision Information Resources Centers sadly reports the death of David Reimer, 38, who took his own life in Winnipeg last week.

David, originally named Bruce, suffered the tragic loss of his penis after a medically unnecessary circumcision caused by physician ignorance of normal penile development in childhood. Psychologists recommended that he be castrated and raised as a female. He was renamed Brenda. At puberty, he was given feminizing hormones to promote breast development and was to be subjected to sex-reassignment surgery, but an attempted suicide caused his father to tell him the truth about his true gender and the botched circumcision. Upon being told, his first question was “What was my name?”

This experiment, an attempt to overcome nature by nurturing the child into another gender proved to be a disastrous failure. When he discovered his true identity, he insisted that he wanted to live as a male, and he chose the name David. He had a double mastectomy and surgery to construct a phallus. He later married and adopted his wifeÂ’s children.

Author John Colapinto wrote As Nature Made Him: The Boy Who Was Raised as a Girl, revealing DavidÂ’s tragic experience to the world. His horrific story has helped to reform medical practice.

David Reimer had the courage to appear on several talk shows, including The Oprah Winfrey Show, where his healthy and robust twin brother was introduced as a member of the audience.

His life took a turn for the worst in recent years. The death of his twin brother and a divorce weighed on his mind, in addition to the undoubted tragedy of his own life caused by the circumcision. He resolved to end a life of unrelenting pain.

David managed to triumph by reclaiming his gender and, by telling his story, protecting others from similar tragedy. He was an inspiration to us all. He long will be remembered as an example of the risk of terrible mutilation caused by circumcision.

Statement of Need:
Morbidity and mortality rates are global measures of a population's health status as well as indicators of the quality of healthcare provided. It is well recognized that the prevalence rates of medical comorbidity and mortality are higher among persons with serious mental illness (SMI) compared to that of the general population. Recently published surveillance data from several state mental health agencies identified that persons with mental illness have 1.2 to 4.9 times higher mortality rates than the population at large. In light of these dramatic statistics, integration of mental healthcare and general physical healthcare is essential to reduce cardiovascular morbidity and mortality in persons with SMI. Recognizing the need to effect change, the National Association of State Mental Health Program Directors (NASMHPD) Medical Directors Council recently published their 13th Technical Report, encompassing recommendations for change in clinical practices as well as those that target policy and organizational structural changes. These recommendations were intended to serve as a clarion call to clinicians, focusing them on the need to integrate behavioral and primary care into mental healthcare.

In addition to educating psychiatrists and other healthcare professionals on current data regarding medical comorbidities in mental illness and the use of antipsychotics, this activity will review recommendations and guidelines that support changes in clinical practice paradigms, as well as approaches and models that have been successfully implemented to integrate mental and physical healthcare. The faculty will also discuss recommendations for changes in the policy environment and regulatory structures that will facilitate organizational support for the integration of physical healthcare into psychiatry.

Today's patients and their families are struggling with serious addictions, co-occurring psychiatric disorders, and significant medical comorbidities. They need help to heal the physical, emotional, and spiritual damage resulting from the disease of addiction. Studies have shown that patients with alcohol dependence and comorbid psychiatric problems benefit from professionally delivered psychotherapy, pharmacotherapy, and greater intensity of structure.

To accomplish these goals, treatment centers need to focus on individualized, assessment-driven strategies that take place within a multidisciplinary treatment team. Each team needs to establish a common language and adhere to a common, evidence-based treatment modality that allows peer supervision and individual planning for each patient's needs over the continuum of care.

Cohesive leadership is necessary to introduce and carry through major change such as the integration of pharmacotherapy into an abstinence-based program for alcohol dependence. Recommendation by a Professional Advisory Board and acceptance from the Board of Directors can begin the process. Multiple training opportunities then follow to educate staff on the efficacy and clinical usefulness of pharmacotherapy.

This interactive webcast with Q&A focuses on effectively managing change in a large treatment center to promote improved outcomes for patients with alcohol dependence.

Americans are fond of using decorative tattoos as a form of expression. According to a study in the September 2006 edition of the Journal of the American Academy of Dermatology, nearly one in four Americans report being "tattooed" at least once. Yet, the same study reports that 17% of those with tattoos would like to have them removed. The good news is that laser treatments and equipment have kept pace with the rising interest in tattoo removal. While there are several lasers that may be used to remove decorative tattoos -- the Q-switched ruby laser -- is the most effective, according to the American Academy of Dermatology.

Whether it's an old flame's name on the bicep or a prom-night red rose on the ankle, Americans are fond of using decorative tattoos as a form of expression. And, notwithstanding the advent of semi-permanent and temporary inks, men and women in the U. S. are choosing permanent tattoos more now than ever. According to a study in the September 2006 edition of the Journal of the American Academy of Dermatology, nearly one in four Americans report being "tattooed" at least once. Yet, the same study reports that 17% of those with tattoos would like to have them removed.

"In our society, tattoos are equated with a youthful free-spiritedness we all value," explains Joshua Fox, MD, founder of Advanced Dermatology and a spokesman for the American Academy of Dermatology. "And with higher quality inks and equipment being used by tattoo artists today, many people are intrigued by the more creative and artistic designs that are available." But, after the painstaking process of choosing a design - and the often-painful process of having the tattoo applied - what prompts those with tattoos to seek solutions for removal? Dr. Fox explains, "Obviously, tattoos that include names of past love interests do not fade, even when the 'old flame' flickers out." He adds, "On a more general basis, people who find tattoos adventurous in their teens and twenties often find that they send the wrong message about them as they move into a more mature stage of their lives, when marriage, family, children and work become the focal point."

The good news is that laser treatments and equipment have kept pace with the rising interest in tattoo removal - which some anecdotal reports estimate is as high as 40-50%. While there are several lasers that may be used to remove decorative tattoos - the Q-switched ruby laser - is the most effective, according to the American Academy of Dermatology. "The Q-switched lasers are the gold standard in removing pigmented skin lesions, which is essentially how a tattoo is identified by the body, this includes short pulse Ruby, Alexandrite, 532 and 1064" Dr. Fox explains. "The ruby laser is effective at breaking down the melanosomes where the ink is stored in the skin into smaller fragments, which are then removed by the patient's immune system," he adds.

Five things to consider before you tattoo:

While Q-switched ruby laser therapy is effective at removing permanent decorative tattoos safely and without scarring, Dr. Fox offers five caveats for those considering obtaining a tattoo:

1) There are possible health risks associated with tattooing, which is largely unregulated in the U. S. Bloodborne diseases are a key concern, as are infections. "Be sure to get references from the artist's most recent clients, and discuss the safeguards that are in place at the studio to reduce or eliminate health risks," Dr. Fox advises.

2) Tattoo removal is not a "same day" procedure, but rather a process that includes the laser treatment session, as well as healing time and any follow-up treatments that might be necessary. "Patients can expect the healing period to be similar to that of any wound, with possible scabbing, burning or itching at the site for one to two weeks after each treatment," Dr. Fox explains.

3) The time - and cost-factors of tattoo removal can be a bit surprising to some patients, depending upon the number of sessions needed to remove the full tattoo. It usually takes 5 - 10 months, for a full tattoo removal, and the costs depending on the site can run into the thousands of dollars. "Tattoos that can take multiple sessions include those that cover a large area of the skin, are intricate in detail, and/or contain various color pigments," Dr. Fox notes.

4) Some tattoos are more "removable" than others, based on the colors of inks used and the tone of the patient's skin. "Fair skin, in general, responds better to laser tattoo removal than darker skin," Dr. Fox points out. In addition, ink colors like yellow, lavender and orange are more difficult and time-consuming to remove, adding more treatments and expense to the tattoo removal.

5) New inks are becoming available that make tattoo removal even easier, because they encapsulate the dye in tiny bio-friendly beads that can be effectively broken down by the Q-switched laser in a single treatment. However, they remain intact like standard tattoo inks, without fading or bleeding, unless and until they are removed with a laser. "For those who are considering a permanent decorative tattoo, but want the option of a simpler, more cost-friendly removal, these new inks may be an ideal option," Dr. Fox concludes.

Uninsured hospital patients routinely pay more for medical services than insured patients, charges that are "unfair, illegal and unconscionable," according to a lawsuit to be filed in circuit court in Broward County, Fla.

The proposed class action lawsuit charges Tenet Healthcare Corp. with charging higher rates to patients who have no insurance coverage than to those with coverage. It claims Tenet rates are twice the industry norm and higher than the actual cost of procedures. This practice violates the Florida Deceptive and Unfair Trade Practices Act (FDUTPA) and the Florida common law of unfair competition.

The case was recently filed following a hospital visit by the named plaintiff, James Garcia. The Miami resident was injured in a fall. After five hours in the emergency room Garcia received a $7,000 hospital bill, including a $1,400 charge for questionable blood and drug screenings. Had he been covered by health insurance, the hospital in most cases would accept less than one-fifth (approximately $1,500) in full payment of the bill.

The bill and lawsuit highlight the discrepancy between what insured and uninsured patients are charged for hospital care. Like some 43 million Americans, Garcia has no health insurance. The case was filed by Freedland, Glassman, Farmer & Sheller of Weston, Fla., Sheller, Ludwig & Badey and Hoffman & Edelson, of Philadelphia, and Hagans Berman of Boston, MA.

"This case is symptomatic of the epidemic that has come about from big corporate control over healthcare," said Jonathan Shub of Sheller, Ludwig & Badey. "If you happen to be one of the fast-shrinking majority of Americans with major medical insurance, your insurance company is charged one set of rates. If you don't have insurance, you personally pay a much higher rate."

The problem stems from several issues. First, insurance companies are able to negotiate deep service and pricing discounts by promising to steer subscribers to facilities on their plans. Also, patients upon admission often are made to sign Conditions of Service or admission agreements acknowledging they will pay a facility's "regular" rates for service. Many patients are signing such documents in pain or under duress. Moreover, when patients have not paid, Tenet has used questionable collection methods, including making negative credit reports.
