Corps of Engineers Awards Stimulus-Funded Construction Contract for Green/Duwamish Ecosystem Restoration

The U. S. Army Corps of Engineers, Seattle District, awarded an American Recovery and Reinvestment Act contract Sept. 16 to Performance Systems Inc., a small business based in Fruitland, Idaho, to begin construction of the Upper Springbrook Creek Ecosystem Restoration Project in Renton, Wash. The $649,990 Upper Springbrook Creek project will restore a 900-foot stream currently running through a roadside ditch along the north side of South 55th Street in Renton.

The U. S. Army Corps of Engineers, Seattle District, awarded an American Recovery and Reinvestment Act contract Sept. 16 to begin construction of the Upper Springbrook Creek Ecosystem Restoration Project.

The $649,990.00 contract funds the construction phase of the project. The Upper Springbrook Creek project will restore a 900-foot stream currently running through a road side ditch along the north side of South 55th Street in Renton. By building a new channel, the project will maximize the habitat and spawning potential for this section of stream. The project also will include a new culvert under South 55th Street to aid in fish passage. The Upper Springbrook Creek is one of the few remaining spawning areas for coho in the Green/Duwamish River watershed.

Performance Systems Inc., a small business based in Fruitland, Idaho, received the award, announced Sept. 16.

The City of Renton is the local sponsor for the project.

"Springbrook Creek is a wonderful asset winding through Renton and by restoring the upper section of the creek we are protecting our salmon habitat and improving the health of our watershed," said Jay Covington, chief administrative officer, City of Renton.

The project team anticipates starting construction in the spring of 2011.

Green/Duwamish Ecosystem Restoration Project
This project is a comprehensive restoration program for the entire Green/Duwamish ecosystem spanning the tidal estuaries to the spawning and wildlife habitat areas in the upper basin. The Upper Springbrook Creek Park project is one of 45 separate projects that emphasize the restoration of critical habitat within the Green/Duwamish River watershed.

Leading food safety lawyer Fred Pritzker calls on U. S. regulators to add non-0157:H7 forms of E. coli to its list of adulterants in ground beef. Currently, only E. coli 0157:H7 is designated as an adulterant in raw ground beef. Other strains of E. coli, such as E. coli 0111, are just as virulent and deserve equally stringent regulation.

Fourteen years after the United States designated E. coli O157:H7 as an adulterant in raw ground beef, one of the nation's leading food safety lawyers is calling on regulators to adopt the same zero-tolerance policy against other strains of the potentially deadly bacteria.

Fred Pritzker, a Minneapolis attorney whose firm specializes in major food-borne illness cases, said several other types of Shiga-toxin producing E. coli (STEC) must be declared adulterants by the U. S. Department of Agriculture to help reverse dangerous upward trends in illnesses and recalls of tainted meat.

Last year alone, E. coli O157:H7 contamination prompted 21 recalls of ground beef totaling 29 million pounds - the highest level in five years. Ten of the recalls were associated with sickness. For a country that purports to have the safest food supply in the world, there's been no progress made since 2004 in wiping out E. coli infection, Pritzker said. In fact, lately the bug has been gaining ground.

"It makes no sense that we are regulating one deadly pathogen and ignoring others," said Pritzker, who noted that one person was killed this summer and more than 300 others were sickened by E. coli 0111 linked to the Country Cottage restaurant in Locust Grove, Okla. The restaurant reopened Saturday after being shut down for an extended period.

In October 2007, the USDA's Food Safety and Inspection Service (FSIS) co-hosted a public meeting at which non-O157:H7 STECs were a leading topic of concern. More than 100 non-O157:H7 strains exist around the world, but health officials in the U. S. are most concerned with O111, O26, O103, O45 and O145.

Six months after the public meeting, the FSIS said it was considering designating certain non-O157:H7 STECs as adulterants. Pritzker said the major policy change is needed right away. "We need to have the same stringent approach to all pathogens that are capable of causing serious harm or death," Pritzker said.

According to the Centers for Disease Control and Prevention, it's possible that non-O157 STEC infections are as prevalent in the United States, or more so, than O157:H7 infections. Since 1990, the U. S. has recorded at least 23 outbreaks of infectious disease caused by the non-O157:H7 micro-organisms. And Like E. coli O157:H7, certain non-0157 STECs can lead to hemorrhagic colitis and hemolytic uremic syndrome (HUS), a condition that has killed children, the elderly and persons with weak immune systems.

By designating as adulterants the most virulent non-O157:H7 STECs, Pritzker said, the FSIS will gain the power to effectively shut down plants when meat is contaminated. The scrutiny will force greater responsibility on the beef industry to detect the bacteria and to prevent it from entering the food supply.

When E. coli O157:H7 was legally labeled an adulterant in 1994, the industry improved its testing mechanisms. Likewise, screening would readily improve for non-O157:H7 strains. The zero-tolerance would also apply to beef imports and provide consumers with stronger protection in recall situations

Proteins Improve Triathlete's Performance

Niggling injuries that flared up just as he was preparing for a major event were the bane of triathlete Martin Cain's life. A regular top ten finisher, in 2007 he won his first competition - the Ripon Triathlon. Then, frustratingly, his training was curtailed once again. But for Martin the win underlined that he was missing out on his full potential.

A year later and Martin's already won two events, at Chester and Horwich, and he hasn't been out of the top five all season. He's had no injuries and he's made significant improvements in his PBs for all three disciplines. So what's changed?

"I was training as hard as I could and, while my diet was healthy, I figured that nutrition might be the key," says Martin, 32, who works as a fireman in Greater Manchester. "So I went to a sports nutrition seminar. Afterwards I collared the speaker, Kerry Kayes, and we chatted about my injuries. He went through my diet and said I wasn't getting enough protein. It made sense, so I started taking protein supplements. That was the turning point."

Kerry Kayes is the sports nutrition specialist who works with several top endurance athletes including boxer Ricky Hatton. You might think working fireman's shifts would play havoc with a triathlete's training and diet, but not a bit of it. "It's the best job I could be doing - I get to train every day, while other people are at work, and I can keep a close eye on my diet, too," he says. "I live nine miles from the fire station, so I run or bike in every day."

"In this study we have evaluated the use of blue light (peak at 415 nm) and a mixed blue and red light (peaks at 415 and 660 nm) in the treatment of acne vulgaris. One hundred and seven patients with mild to moderate acne vulgaris were randomized into four treatment groups: blue light, mixed blue and red light, cool white light and 5% benzoyl peroxide cream. Subjects in the phototherapy groups used portable light sources and irradiation was carried out daily for 15 min. Comparative assessment between the three light sources was made in an observer-blinded fashion, but this could not be achieved for the use of benzoyl peroxide. Assessments were performed every 4 weeks. After 12 weeks of active treatment a mean improvement of 76% (95% confidence interval 66±87) in inflammatory lesions was achieved by the combined blue±red light phototherapy; this was significantly superior to that achieved by blue light (at weeks 4 and 8 but not week 12), benzoyl peroxide (at weeks 8 and 12) or white light (at each assessment). The final mean improvement in comedones by using blue±red light was 58% (95% confidence interval 45±71), again better than that achieved by the other active treatments used, although the differences did not reach significant levels. We have found that phototherapy with mixed blue±red light, probably by combining antibacterial and anti-inflammatory action, is an effective means of treating acne vulgaris of mild to moderate severity, with no significant short-term adverse effects."

Pharmacogenetics, the study of influence of genetic factors on drug action and metabolism, is used for predicting adverse reactions of drugs. Several enzymes are involved in drug metabolism of which the most important ones are those belonging to the family of cytochrome P450. The knowledge of the effects of polymorphisms of genes for the enzymes is applied in drug discovery and development as well as in clinical use of drugs. Cost-effective methods for genotyping are being developed and it would be desirable to include this information in the patient's record for the guidance of the physician to individualize the treatment. Pharmacogenomics, a term that overlaps with pharmacogenetics but is distinct, deals with the application of genomics to drug discovery and development. It involves the mechanism of action of drugs on cells as revealed by gene expression patterns. Pharmacoproteomics is an important contribution to personalized medicine as it is a more functional representation of patient-to-patient variation than that provided by genotyping

Biological therapies such as those which use patient's own cells are considered to be personalized medicines. Vaccines are prepared from individual patient's tumor cells. Individualized therapeutic strategies using monoclonal bodies can be directed at specific genetic and immunologic targets. Ex vivo gene therapy involves the genetic modification of the patient's cells in vitro, prior to reimplantation of these cells in the patient's body.

Various technologies are integrated to develop personalized therapies for specific therapeutic areas described in the report. Examples of this are genotyping for drug resistance in HIV infection, personalized therapy of cancer, antipsychotics for schizophrenia, antidepressant therapy, antihypertensive therapy and personalized approach to neurological disorders. Although genotyping is not yet a part of clinically accepted routine, it is expected to have this status by the year 2010.

Several players are involved in the development of personalized therapy. Pharmaceutical and biotechnology companies have taken a leading role in this venture in keeping with their future role as healthcare enterprises rather than mere developers of technologies and manufacturers of medicines.

Ethical issues are involved in the development of personalized medicine mainly in the area of genetic testing. These along with social issues and consideration of race in the development of personalized medicine are discussed. Regulatory issues are discussed mainly with reference to the FDA guidelines on pharmacogenomics.

Increase in efficacy and safety of treatment by individualizing it has benefits in financial terms. Information is presented to show that personalized medicine will be cost-effective in healthcare systems. For the pharmaceutical companies, segmentation of the market may not leave room for conventional blockbusters but smaller and exclusive markets for personalized medicines would be profitable. It is not all inclusive, and the author invites the reader to use the book as a springboard in the search for answers to some of the mysteries of sex, disease, health, and longevity.

The forum, held in the STAR (Skills Training, Academics and Resources) Center, will bring together industry leaders to better understand the requirements of USP 797 and their practical application for facility design and environmental control.

Baxa Corporation announces plans to bring together industry thought leaders for a day-long forum on cleanroom compliance and best practice. Active members from the United States Pharmacopeia Council of Experts, the American Society for Healthcare Engineering (ASHE) and Colorado Association of Healthcare Engineers and Directors (CAHED) and the Colorado Board of Pharmacy are invited to the session to discuss common goals and challenges in meeting USP "cleanroom" or 797 requirements. The first class, planned for January 25, 2008, is an invitation-only event. Following this forum in January, the course will be open to outside enrollment. Initial program dates are October 24, 2008 and November 14, 2008.

This extension of the Baxa STAR Center course offerings - Facility Design & Engineering Controls for USP 797 - is a new design course intended to provide an in-depth educational resource regarding compliance with USP 797 for facilities managers, engineers and architects. The Facility Design & Engineering course brings the realities of process flow, work flow, and raw material flow together with the cleanroom design requirements set by USP 797.

The goal of the inaugural course is to facilitate understanding on idealism vs. the realities of USP compliance among those who comply with and enforce the regulations. According to Pete Campanella, Baxa Director of Professional Services, "Baxa is excited to be part of this forum. Bringing these groups together promotes open communication and cooperation to further best practices in the hospital pharmacy." The open forum will serve as a catalyst for these organizations to join in helping hospital pharmacies and facilities managers build and maintain the necessary facilities to improve the quality of parenteral care delivered to patients.

Course topics for Facility Design & Engineering Controls for USP 797 include aseptic technique and movement in the cleanroom, engineering controls required for hazardous and non-hazardous drugs and certification procedures. Through these topics, participants will gain a thorough understanding of the scope of USP Chapter 797, the facility requirements defined in USP 797 and the use of smoke studies to validate proper airflow.
