A named-patient program allows physicians and their patients access to drugs which have not yet received approval for marketing by national health authorities. "European Named Patient Programs, like US compassionate use programs, offer physicians access to pharmaceuticals which have not yet been licensed" explained Dr. Emmer, "However, there is one important difference: in Europe an unlicensed drug is often purchased by National Health Systems." This presents drug-makers with an opportunity to generate revenues while development is still in-progress.

Significant Revenues Are Possible

The additional revenues can be considerable. For example, a source at Pharmion, a US based company focusing on Oncology and Hematology reported dramatic increases in its Thalidomide sales from $1.9 million in 2Q ‘03 to $15.3 million in 2Q ‘04, primarily due to named patient sales in Europe for Multiple Myeloma. Thalidomide sales accounted for approximately 75% of Pharmion’s total revenues for the first half of 2004, according to company sources, and were generated while the product awaits marketing approval for this indication. Before receiving European Marketing Approval, Shire’s Argylin® for essential thrombocythaemia generated about 5% of its total sales from its European named patient program.

Other Benefits of Named Patient Programs

A named patient program can speed uptake after official launch. Physicians, who have had experience before launch, via clinical trials or named patient programs, often become early adopters and references for other physicians once the drug is freely circulating.

Named patient programs, like US compassionate use programs, can increase good-will toward the company because they simplify the process of gaining access for patients in critical need. Smaller companies often can not afford the administrative time and costs of shipping drugs around the world before launch. This can lead to frustration and resentment towards a company that many physicians will remember long after a drug is officially on the market. Creating a formal channel eliminates the unfortunate need of denying requests and risking ill-will later.

A named patient program should be considered an important part of a pre-launch program. It increases awareness to a pharmaceuticalÂ’s existence, creates excitement, generates good-will and speeds penetration of the product after launch.

Frequent Communication is Necessary

If one of the objectives is to generate revenues, setting up a named patient program is just the beginning. In order to achieve success, physicians need to be aware of the product and what they need to do to get it. Typical methods of informing physicians, such as sales rep visits and ads, may not be appropriate because a license is necessary to market a drug. While physicians are used to simply writing a prescription and being done with it, named patient programs require paper-work that some find tedious. Therefore the company needs to create an appropriate communication plan and work closely with the targeted medical community to keep them informed and simplify the process.

Payment Plans: Because this art is predicated on follow-up treatments, consumers should make sure the practitioner offers a package deal which is reasonable for these services. If it is not offered, make sure to ask for it. It is within reason of good customer service.

Timing: Patients should not have treatment for facial rejuvenation more than twice per week, safely. Any more than twice per week can lead to symptoms like headaches, dizziness, insomnia, nervousness.

Experience: Is the practitioner credentialed? How do you know? Are their credentials current and available for you to see? What is their stated area of expertise? All licensed acupuncturists should be credentialed by the National Certification Commission for Acupuncture and Oriental Medicine (NCCAOM).

This sequel to the original 'Developing the Business Case for ITIL' makes the point that even though less than half of today's businesses are developing business value and ROI to justify IT spending, that trend is changing quickly. Going forward, IT departments must develop business value justification and ROI to fund new projects. This is evidenced by increasing pressure on IT executives to not only justify investments via traditional internal metrics (such as numbers of servers per system administrator) but also to expand into metrics that quantify IT's value to the business, such as 'annual contribution' to the business, 'benefit per developer' and other types of metrics, such as increasing IT staff time on innovation versus operations.

Templates are offered for capturing specific metrics for Incident and Problem Management; Change, Release and Configuration Management; and Service Level and Financial Management. “Crystallize”, designed by Paul Cocksedge for the Swarovski Crystal Palace Collection, is a chandelier made from light. When there is no light the chandelier disappears. Using lasers and crystals in perfect alignment, the iconic shape of a Swarovski crystal is drawn in mid air. At the heart of the chandelier is a single crystal which sparkles as it catches the light.

The development of this chandelier was made using Photonic ProductsÂ’ green laser modules.

The 532nm DPSS green laser module is a compact, self-contained module designed for use where a highly visible beam is required, primarily in alignment and also machine vision, industrial inspection and scientific equipment.

It produces a very visible circular output beam of less than 2.0mm diameter and output powers of 0.9mW and 4mW with a maximum operating current of 250mA. Power stability is better than 5% over 7 hours. Modulation capability of 0-1kHz is standard. In addition, the module uses a front facet monitor photodiode to maintain a stable output power, as opposed to using the internal monitor of the laser diode, which is often less responsive.

What is needed, according to Anssi Manninen, a leading authority on hard-core sports nutrition, is an entirely new elite class of ultra-hydrolyzed proteins that can be absorbed directly into the bloodstream without going through the standard digestion process. Manninen’s research, published in an article in the prestigious British Journal of Sports Medicine, asserts that only these rapid-acting proteins are capable of reaching muscle tissue swiftly enough to maximize muscle anabolism during a critical post-workout “anabolic” window of opportunity which exists for as little as 90 minutes after exercise.

Needless to say, these surprising revelations have left athletes wondering where to turn for the ultra-assimilatable proteins they require to fuel real mass gains.

The result? Blood insulin responses were 66% and 221% greater in the healthy controls in the carbs-plus-protein-hydrolysate and carbs-plus-protein-hydrolysate-and-free-leucine trials, respectively, compared with those in the carbs-only trial. In other words, this study demonstrated that co-ingestion of a protein hydrolysate strongly augments insulin secretion (a key component of muscle mass accrual and maintenance).

In another study, conducted on protein hydrolysates at the Copenhagen Muscle Research Center, it was determined that “whey protein hydrolysate elicited the greatest availability of amino acids during the three-hour postprandial (occurring after a meal) period. This difference was attributed to the rapid increase in blood amino acids evoked during the first 40 minutes of the digestive period, during which the increase was about 37% greater after the ingestion of whey protein hydrolysate solution than that after ingestion of the intact milk protein solution.” The authors suggested that the association of high levels of blood amino acids and insulin might explain a superiority of protein hydrolysates over intact proteins in promoting better nitrogen utilization (i. e., greater anabolism), especially when administered in combination with high-glycemic carbohydrates.

The importance of these studies for athletes is unmistakable. Ultra-hydrolyzed proteins show great promise as a trigger for increased insulin response and greatly enhanced delivery of key aminos to nutrient-starved muscle tissue.

Clearly, the twin sciences of protein formulation and muscle-mass enhancement have entered into a period of great transformation. Advanced products such as NutraQuest’s MyoZene—as well as other competing formulations soon to be introduced—represent the wave of the future. The time of bulky, slow-acting protein formulations has come and gone.
