While the nationally-recognized ACOEM disability treatment guidelines are already available on MDGuidelines (www. mdguidelines. com), states often develop their own specific guidelines to address the most costly and/or common workplace injuries for the prevalent industries in that state. State treatment guidelines typically outline what will and won’t be reimbursed for medical providers handling workers’ compensation injuries with the intention of expediting treatment and lowering costs by limiting unnecessary or unproven treatments.

The Washington State Treatment Guidelines are arranged by medical topic (each of which encompasses multiple medical codes). The Colorado Guidelines are arranged by 10 regions of the body (such as “lower extremity”), each covering numerous medical topics.

While both sets of state guidelines are available as downloadable PDF documents from their respective state websites, MDGuidelines greatly improves functionality, access and usage with the ability to easily search and navigate the content. Icons signify whether a procedure is recommended, not recommended, or should be approached with caution (meaning that there may be a list of objective data requiring documentation for the procedure to be approved). Close to 1,000 Attend National Black Religious Summit on Sexuality

Washington, DC – Determined to “break the silence” about sex and sexuality, nearly 1,000 people from interfaith communities across the country assembled recently at Howard University’s School of Divinity for inspirational worship, self-reflection, and dialogue on faith based sexual education during the National Black Religious Summit VII on Sexuality.

For the past seven years, the Black Church Initiative of the Religious Coalition for Reproductive Choice (RCRC) has hosted the three-day Summit to empower clergy, congregations, parents and guardians, with the skills to dialogue with teens about sex and sexuality.

“Faith communities must address the critical health issues that impact the lives of young men and women and help them to make sound choices when faced with life-threatening, life altering sexual decisions,” said Reverend Carlton W. Veazey, President and CEO of the RCRC.

Each morning worship services in HowardÂ’s Thurman Chapel were broadcast, with sermons from Rev. Dr. Jeremiah Wright of Trinity United Church of Christ in Chicago, Rev. Mary Moore of New Salem Missionary Baptist Church in Memphis, TN, and Rev. Dr. Alvin O Jackson of National City Christian Church in Washington, DC, setting the tone for a spirit-filled day.

With the theme “Breaking the Silence: In Times Like These,” the Summit addressed teen pregnancy, HIV/AIDS, sexual addition, and other reproductive health issues within the context of African American religion and culture. Workshops took a critical look at sexuality in the scriptures, and explored issues like homophobia, depression, and effective family planning methods. A workshop was also conducted in Spanish for Latino/Hispanic congregations.

Youth sessions, first established in 1999, gave teens the opportunity to dialogue about peer-to-peer conflict resolution, responsible sexual behavior, and effectively communicating with adults. They also developed a Vision Statement for their peers and pre-teens about how their faith is preparing them for the plans that God has for their lives.

“It was great to be able to speak openly with adults about the pressures we experience daily and talk about what the Bible says about issues we are confronted with,” said a 15-year-old youth from Victory Baptist Church in Decatur, Georgia. “It also helps to meet so many like-minded people, it let’s me know that I’m on the right track,” she adds.

The RCRC launched the Black Church Initiative in 1997 to “break the silence” about sex in African American churches. Many of the Summit participants have implemented programs of Black Church Initiative for their local congregations. Programs include, “Keeping It Real!,” a seven week sexuality education dialogue model for teens, “Breaking the Silence,” a five week dialogue model for adults focused on strengthening relationships between teens and adults, and “Now We Can Talk,” an intergenerational dialogue model for teens and adults.

New study by Billtrust and IDEXX Laboratories highlights preference for electronic billing.

Billtrust, the leading billing service provider, announced results of a billing survey conducted in conjunction with IDEXX Laboratories. The goal of the study was to measure satisfaction levels among IDEXX customers for their bill delivery method and bill design. The results of the research highlight the importance for B2B companies in understanding the different bill delivery methods available.

Over 1000 IDEXX customers completed the survey in 4Q 2008. The conclusions indicate a high level of preference for electronic bill delivery methods. Among IDEXX customers who were already receiving their bill by email, 95% said they prefer this method. This compares with 77% of IDEXX customers getting their bills by US Mail saying they prefer US Mail for receiving their bills. The large majority of U. S. Mail customers who said they would prefer an alternate method stated a preference for email or online billing.

According to Melissa Seader, Senior Manager Accounts Payable/Accounts Receivable at IDEXX, "While the goal of the survey was to evaluate ways to improve our billing process, we were pleased to be able to transition many of our customers to electronic billing." Seader continued, "We were able to satisfy more of our customers, reduce our billing costs, and do some good for the environment."

Other findings of the survey further emphasized the role electronic billing can play:
 The primary drivers of preference for email bill delivery include "Ease of Keeping Organized" and "Timing to Receive Bills". B2B bill recipients say they want to get their bills faster so they can 1) take advantage of early payment discounts, and 2) bill their own customers faster.  Electronic bill recipients are twice as likely as US Mail recipients to pay their bill online. For billers, this indicates a pathway towards further cost reductions. By migrating more customers to electronic bill delivery, businesses will be better positioned to transition them to electronic payment.  Over 60% of IDEXX customers are using QuickBooks and Peachtree accounting systems. IDEXX can use this data to drive electronic adoption even further by offering these clients the ability to download their data directly into these accounting systems. This is done using Billtrust's EasyImport technology. "The IDEXX results are consistent with the conclusions from surveys we have conducted with our other clients," said Mitch Rose VP Marketing at Billtrust. "Many of us tend to focus on driving eBilling because of the significant cost savings and the environmental benefits, which are both very important. "As our diet popularity increases, we receive lots of questions concerning the principles it is based upon. Let me give a couple of very simple definitions to clarify the matter," says Tanya Zilberter, PhD. "There's nothing mysterious at all.

The fat burning rate is the number of milligrams of fat burnt by 1 kg of body fat in 1 minute. Our Fat Burning Index is just a term; we use it for quantitative description of foods' and meals' potentials to burn or to store fat."

The point can be best explained in terms of ketosis, which any low carb dieter is familiar with. Ketosis is a biochemical condition that occurs when the body produces ketones (ketone bodies) at a higher rate than glucose's release into the blood.

Ketone bodies provide energy for the body's needs when there is a total food restriction, carbohydrate restriction, and/or long enough physical effort.

In the beginning of ketosis, ketones provide up to 75% of total energy requirement, including energy required by the brain. Most dieticians say that carbohydrate is the only fuel for the nerve cells but it is not true. It's a preferred fuel, yes, but not the only one. If there are little or no carbohydrates, all tissues of the body including brain tissue can adapt to using alternative fuel.

As the body undergoes the process of adaptation to ketosis, the amount of carbohydrates required reduces. This explains why brain fog and muscle weakness can occur in the beginning of ketosis but becomes very rare after a week or two. In fact, it's true that a diet is considered low carb if it contains less than 100 carb grams a day, but it's only true until the adaptation to ketosis develops. After it is completed, the carb requirement is around 40 grams. None of non-clinical low carb diets takes this fact into account.

After 1-2 weeks, as the process of adaptation to ketosis develops and the tissues no longer rely on carbohydrates for fuel, they send less signals requesting the beta-cells in the pancreas to release insulin -- resulting in decrease of insulin concentration in the blood.

Low insulin level frees its antagonistic fat burning hormones from suppression resulting in increased levels of glucagon, growth hormone, catecholamines, and glucocorticoid.

Glucagon is the most important insulin antagonist; it's up to glucagon to control fatty acids' release from fat stores to be burnt for fuel. So, as glucagon release increases, insulin goes down. It is thought that to initiate ketosis, carbohydrate intake should be reduced to less than 30g/day.

"There's a most important condition grossly overlooked by the authors of low carb diets," warns Dr. Zilberter. " Any food is either ketogenic or glucogenic depending on it's influence on the competition between glucagon and insulin. To make ketosis possible, a meal should contain at least 1.5 g of fat per every 1 g of protein plus carbohydrate combined.

Only that or higher of a ratio makes food ketogenic enough to allow eating without portion and calorie control. Foods with ketogenic indexes below 1 promote insulin release and are essentially anti-ketogenic. Foods with indexes between 1 and 1.5, though not anti-ketogenic, require calorie control since their ketogenic properties are not strong enough to significantly suppress the carbohydrate metabolic pathway and mobilize body fat for fuel thus causing healthy hunger decrease."

Adults Taking Eplileptic Medications More Likely to be Unemployed

A study published in the journal Epilepsia shows higher unemployment and a lower health-related quality of life (QoL) in patients who had epilepsy as a child.

“Strikingly, in self reports, the health-related quality of life was not determined by good or bad psychosocial function or having, or not having, seizures, but rather by whether or not they were taking long-term antiepileptic drug therapy,” said lead researcher, Matti Sillanpää of the University of Turku.

Researchers revisited a group of 99 adults, originally from a group of 245 pediatric patients, 30 years after having active epilepsy. 67% were in remission off medication while 14% were still taking antiepileptic drugs (AED). However, the study found that whether or not they were in remission, those patients still taking long-term AED therapy showed higher rates of unemployment and lower socioeconomic status. Subjects off medication had results similar to those of the control group. Both groups had lower rates of marriage and having had children than the control. The study notes that differences in employment and socioeconomic status are not attributable to differences in educational status.

“Maybe the most important feature [of this study] is that it is a very long-term follow-up period of children with epilepsy, and children with uncomplicated epilepsy in particular, which allows the examination of the effects of epilepsy itself on the whole life of an individual,” added Dr. Sillanpää. “Similar recent studies are not available.”

As evidenced by the study, childhood-onset epilepsy has a long-term adverse effect on QoL and being on medication may be a marker for other adverse effects, such as stigma.

When You Don't Fit the Mold…Make a New One is the title of a new book from the Massachusetts Medical Society containing the personal stories of 22 women physicians -- the decisions they made, the roads they traveled, and the obstacles they faced on their way to and through medical school and on to a professional career as a medical doctor.

Published on the 25th anniversary of the Society's Committee on Women in Medicine last month, the book offers a revealing look at the experiences of 22 women whose medical schooling and practice eras span more than six decades. Formed in 1981, the Committee serves as an advocate for women physicians and provides a forum for networking, mentoring, and leadership development.

"Not one of us did exactly what our families or society expected of us," said Mary Kraft, M. D. in her introduction to the book. "Each woman created a unique mold for herself. Perhaps the only common threads are the passion and dedication we bring to medicine and patient care." Dr. Kraft, chairperson of the Committee on Women in Medicine from 1997-2001, in part provided the initiative for a conference on women in medicine that eventually led to the book.

Edith Jolin, M. D., a psychiatrist and current chairperson of the Committee on Women in Medicine, says women have made great strides in medicine in recent years. "We've progressed from the days when medical school classes had but a handful of women to today, when half or more of the entering students are women." But she adds that physicians face the same pressures as other professionals in demanding careers. "Every working family is trying to figure out how to achieve balance and a productive, healthy life."

Claudia Koppelman, M. D., a Holyoke physician who co-chaired the Committee when the book initiative began, wrote in the preface, "The experiences of these women, the decisions they made, and the opportunities they chose to follow can be attributed to their individual strength of character. The knowledge that conformity is unnecessary in the quest to become a skilled physician was the key that allowed these women to pursue their dreams."

The chapter titles in "When You Don't Fit the Mold…Make a New One" are indicative of the varied experiences of the women themselves. A sampling: "M. D. - Mother Died or Marvelously Diversified;" "Pregnancy During Fellowship;" "Achieving Balance;" "Doing it My Way, On My Time;" "The Uneven Road;" or simply, "My Story." Three of the women featured in the book have been elected president of the Massachusetts Medical Society, the statewide organization of physicians that today has some 18,500 members.
