"People often prepare for financial emergencies and have the best laid out plans in front of them for when crisis strikes, but all too often those plans fall short and crippling debt becomes the norm."

According to Mr Rhode, concepts such as educating consumers on how to properly manage their finances or warning individuals about the dangers of overspending do nothing to help people who end up in debt due to an overwhelming life event. Mr Rhode said:

"Whenever debt levels rise or the number of bankruptcies increases, there is always a cry that we need more education to teach people how to manage their money, but how does financial education help the single mother with three children who was recently abandoned by her husband? We can put warning labels on credit cards and we can tell people to spend within their means, but without concentrating on solutions for those who are already facing problem debt, we're just spinning our tyres."

Mr Rhode said that giving people the skills and tools necessary to avoid problem debt should be a large priority, but that emphasis also needs to be placed on providing the right solutions when people get into trouble. He said:

"All the financial education in the world can't stop life events from causing people to get into debt. And while education is important to give people the tools and knowledge to manage their money responsibly, we need to concentrate on providing caring, compassionate solutions when people find themselves in over their heads."

In this world of "pay-for-everything", freelancers are not taking things on the chin! Hundreds of 'lance sites are appearing on the internet to offer projects to freelancers for a price. This in itself is setting a dangerous precedent as global competition for these projects is driving the costing down.

The Internet has seen an explosion in so called "lance" sites. Their services are a sort of go-between to bring together clients requiring projects with freelancers seeking work. Whilst the client uses the service for free, the freelancer is charge an ever increasing fees just for the privilege of bidding for the projects. In theory this seems reasonable but in practice the market is being destroyed by lower and lower bids as more freelancers from around the globe bid against each other. This in turn is pushing down the quality of the available work.

Some job sites charge up to £175 a month to access their database of projects. This situation is fast becoming critical for the freelancer. To add to all these problems there are students offering to work for nothing just to build up their portfolio. They don't realise that when they leave college they'll be trying to support themselves on unsustainable market expectations they themselves created.

“Health care will change in the next administration, and the implications will be enormous,” said Sandy Lutz, managing director, PricewaterhouseCoopers’ Health Research Institute. “But this is an incredibly complex issue. No plan is perfect and there will be trade-offs. The Republicans have chosen a different set of trade offs than the Democrats have.”

Changes in policy could dramatically alter the government's clout. The government's share of health spending has been steadily increasing, and it now pays for just less than 50 percent of all health care in the U. S. Current estimates indicate that more than half of all health spending will be government funded by 2017, but it may be even earlier due to expansions of government programs. For example, spending an estimated $100 billion to expand universal access would make 2011 the year when the U. S. will have a primarily government funded health care system.

Other key findings of the report are:
Both parties' approaches may have far reaching consequences for the insurance market with a huge boost to the individual market and some of the first cracks at the historically employer-dominated insurance system. Republicans want to grow the individual market through tax incentives and deemphasize the employer sponsored insurance market, while Democrats want to mandate individual and employer insurance. Either approach is likely to expand the existing private insurance market, which has seen minimal growth recently despite the introduction of tax-advantaged health savings accounts. All of the candidates are talking about decreasing cost and improving quality. Many health industry leaders believe that programs and tools such as comparative effectiveness, information technology and wellness programs are the best hope for improving the efficiency of our current system. However, none of the candidates wants to mandate the use of information technology, and only Senators Obama and Clinton have proposed additional government funding to accelerate adoption by hospitals. Democratic candidates’ proposals to control the cost of drugs, such as direct negotiation for Medicare drugs, drug reimportation (http://www. pwc. com/extweb/pwcpublications. nsf/docid/FE1F5F272DE00D038525739400635686) and bio-generics, have the potential to significantly affect the pharmaceutical industry, which currently accounts for about 10 percent of overall health care spending. All of the candidates have proposed tax law changes to pay for care of the uninsured, equalize the tax benefits of buying health insurance coverage or encourage consumers to purchase health care more appropriate to their needs. The crux of their differences is the candidates’ positions on the Bush tax cuts set to expire in 2010: The Republicans want to extend the tax cuts while the Democrats want to preserve some of them, and spend the rest on health care.

“The Democrats are talking and writing about health care far more than the Republican candidates. But more talk does not necessarily mean more substance. There are still many unanswered questions about the downstream impact of each plan,” added Lutz. “We think all stakeholders of healthcare, and certainly all our clients, need to know how those differences will affect them.”

The report covered candidates who averaged at least 10 percent in national polls for their party’s nomination: Hillary Clinton (D); John Edwards (D); Rudy Giuliani (R); John McCain (R); Barack Obama (D); Mitt Romney (R); and Fred Thompson (R).

The lives of two worthy Azusa Pacific University nursing students have been significantly changed recently because of Pasadena's Convalescent Aid Society (CAS). Rosy Javier and Veronica Dougherty were each honored with a $2,000 scholarship to aid their efforts toward serving others.

Javier, 21, who comes from a family of medical professionals, had to get a part-time job as a behavior therapist to help pay for her tuition. Finding out about the scholarship on the very due date of the applications then was nothing short of serendipitous.

"As a little girl, I (have) always wanted a career in the services - any situation where I could make a difference," said Javier, who worked with the kids on Skid Row as an advisor and mentor during her first two years of high school. "It's so fulfilling," she confirmed, "that I know it's what I want to do with my life." Javier looks forward to getting her master's degree in hospital administration and someday open a free clinic in inner city Los Angeles.

Dougherty, 41, is a wife and mother of 4 kids. Unlike some tireless college-aged students, her obstacles also included the difficulty of re-entering school later in life.

"I was committed to going back to school," she said, "but didn't know how I would pay for it. With the financial assistance that CAS has provided, I'm absolutely grateful beyond words. It has made all the difference."

Specifically, Dougherty expressed a love for helping others in times of distress. "I want to make to make a difference in the lives of those who feel as if they've lost hope, whether it's HIV patients or unwed teenage mothers."

Someday, she hopes that these aims will lead her to different countries to provide health care. "I especially feel drawn to the underprivileged ones that don't have access to it - the people who can't afford health care or live miles away from the nearest hospital."

Cancer has been around as long as mankind. In the second half of the 20th century, the number of reported cancer cases exploded. Upon discovery of a Los Angeles man's need for a heart transplant and his allergy to heparin (a blood thinner that must be used during this procedure to prevent blood clots), Cedars-Sinai Medical Center physicians resort to using a synthetic form of protein found in the saliva of medicinal leeches that, under supervision proves successful in saving his life and wedding plans.

Los Angeles resident Daryl Vinson, 39, desperately needed a heart transplant (http://www. csmc. edu/8603.html), but was allergic to heparin; a blood thinner that plays a critical role in transplantation surgery.

Rather than allowing their patient to languish and die, members of the heart transplant team at Cedars-Sinai Medical Center created an alternative, using a synthetic form of a protein found in the saliva of leeches (http://www. csmc. edu/12698.html). Bivalirudin was recently approved by the Food and Drug Administration for the treatment of certain cardiac conditions, but because it has no antidote and its use in transplantation is so new, the doctors had to develop a game plan and specific protocols in advance of the operation.

Vinson, a former Air Force air traffic controller who also served with the Global Command and Control System, caught what he thought was a common cold that quickly got worse in early June. After suffering a nearly fatal collapse, he was diagnosed with idiopathic dilated cardiomyopathy, a severely weakened, poorly functioning heart.

While dilated cardiomyopathy (http://www. csmc. edu/5513.html) can be caused by a number of factors, including reduced blood flow to the heart ("ischemic" disease), Vinson's condition likely stemmed from a viral infection.

"Mr. Vinson appeared to be in relatively good health, so it was a surprise to everyone, including his physicians, that his heart function was so bad," says Ernst Schwarz, M. D., Ph. D., a specialist in transplantation cardiology.

"The doctors basically told me I was dying," recalls Vinson, who had returned to Los Angeles in January from his hometown of Memphis to direct communications for the Compton school district. During a year back home helping his mother and disabled stepfather, Vinson had fallen in love and become a father.

"I had to call my fiancée in Memphis and tell her I probably couldn't get married in October; that I was dying," Vinson said.

He was transferred to Cedars-Sinai on June 29, and the initial plan was to implant a cardiac defibrillator, says Schwarz. That effort was thwarted by the discovery of a massive blood clot in the patient's left ventricle, the heart's main pumping chamber. A heart biopsy revealed an unusual amount of scar tissue for a patient with such a short history of non-ischemic cardiomyopathy, suggesting limited potential for recovery. A new heart was Vinson's only real hope.

"Here's a guy who was playing football not long ago, who was very successful and looking forward to marriage and family," says Sinan Simsir, M. D., surgical director of Cedars-Sinai's Heart Transplant Program (http://www. csmc. edu/2352.html). "The next week, he has the heart function of someone near death."

Lawrence Czer, M. D., medical director of Cedars-Sinai's Heart Transplant Program, and director of Transplantation Cardiology, concurs: "Mr. Vinson had such an advanced case of heart failure that he was put on the organ wait list almost immediately."

With Vinson's heart working at just 10 percent capacity, his doctors were close to giving him an artificial heart pump to keep him alive, but a heart became available within three-and-a-half weeks, a remarkably short wait. But in his earlier course of treatment, Vinson was found to have an allergic reaction to heparin. Re-exposure during surgery could result in life-threatening clotting complications, says Simsir, so the team would have to use an alternative.

"Heparin is critical in transplantation because it prevents blood clots from forming while the patient is on the heart-lung machine," explains Czer, who says the situation intensified when it was verified Vinson had heparin-induced thrombocytopenia (HIT) syndrome, which prohibited use of any heparin-related blood thinner. This condition affects an estimated one to four percent of patients treated with heparin.

Fortunately, Simsir had previous experience using bivalirudin. Marketed as Angiomax, the heparin alternative is now indicated for use in coronary interventions, including angiograms and stent procedures, and for the treatment of angina (http://www. csmc. edu/5190.html), heart attack, deep vein thrombosis and other blood clots.

There was a problem, though. Typically, a blood thinner such as heparin is followed by a course of protamine to restore the blood's normal coagulation after surgery.

"There is no antidote for bivalirudin," says Czer, explaining the increased risk. "It has to be administered very carefully, and then you have to filter it out of the system while you are transfusing blood products to reduce the bleeding. We built this filtering into our protocol."

Prior to surgery, the transplant team developed new protocols for use of the agent, which took 24 hours to collect in the large quantities required. Bivalirudin presented two distinct challenges, according to Simsir: 1) the lack of an antidote and, 2) no proven track record. He adds that many transplant centers would have refused the case. "Who knows how many patients with HIT are turned down?"

Schwarz believes this encounter with bivalirudin will open the door to more transplant procedures for heparin-sensitive patients. "The agent's use is relatively new in the OR, so this has been a valuable experience for us."

The transplant was completed in a three-hour procedure without complications, thanks largely to bivalirudin, the synthetic form of hirudin, a powerful anticoagulant produced by the salivary glands of medicinal leeches. Vinson went home three weeks later.

"I have a new birthday, you know. It's July 25, 2007 - the day I got my new heart," he says.

Just before surgery, Vinson presented a proposal and ring to his fiancée, Margo McLemore, who had dropped everything in Memphis to rush to his side in Los Angeles.

"I didn't get down on my knees, though, because I wasn't sure I'd be able to get back up," he adds with a laugh.

Also at Vinson's side throughout his ordeal was the man he calls "Pops," Don Ross, a prominent former newsman - one of the first African-Americans in the business - who is now involved in the entertainment industry.

Vinson remembers regaining consciousness after surgery: "I looked around and thought, I don't see angels, I don't hear singing - cool!"

That relief was followed by a flood of emotion and overwhelming gratitude for the gift of his life, Vinson says.

"I was thanking God and crying. I'd never experienced such love, compassion, competence and care. I wondered, what did I do to deserve this? I have a joy that I can't even explain."

Vinson's exhilaration inspired him to "tie the knot" right away, he says. The couple and their witnesses gathered Aug. 10 in Cedars-Sinai's chapel for a small, simple Judeo-Christian ceremony.

Though Vinson faces several months of recuperation and weekly medical checks, his prognosis is encouraging - two biopsies have shown no sign of organ rejection. He looks forward to returning home with his new wife and young son, Joshua, and to resuming the job he'd already learned to love in Compton.

Aside from the physicians and staff who saved his life, Vinson feels indebtedness - and a sense of responsibility - to his anonymous donor.

"I have double to do now, because another life was given to me. Someone died so that I could live, and I need to be worthy of that sacrifice."

Cancer treatment is so physically and psychologically debilitating that nearly 33 percent of patients undergoing chemotherapy quit before completing the regimen.

Cancer treatment is so physically and psychologically debilitating that nearly 33 percent of patients undergoing chemotherapy quit before completing the regimen.

A recent study published in the August 16, 2006 issue of the Journal of the National Cancer Institute drove this point home recently by reporting that in clinical trials the side effects of chemotherapy are woefully underreported. This translates into a devastating outcome for patients undergoing real-world cancer treatment, where they are experiencing more complications than estimated during clinical trials. Anemia, dehydration, malnutrition, infection, fever, reduced white blood cell counts are among the adverse side effects that the authors of the article cite as reasons women undergoing chemotherapy for breast cancer visited emergency rooms. And for each month of chemotherapy the odds of experiencing a serious side effect increased by 20% per month of chemotherapy administered after initial breast cancer diagnosis. Is it any wonder so many patients simply drop out of treatment?

“It is far more common to find a patient exhausted from chemotherapy than to exhaust all the available treatment options,” explains Keith I. Block, MD, Medical/Scientific Director of the Block Center for Integrative Cancer Treatment who, with his wife Penny, developed a Cancer Rehab program that gets patients in shape before, during and after their rigorous cancer treatments.

“Just as a surgeon would prefer a patient be as nutritionally, physically and psychologically fit as possible before undergoing surgery, it follows that a physician treating cancer would want the same for their patients,” Dr. Block says. “The stronger the patient, the better chance they have of withstanding the treatments used to fight their disease.”

In order to offer each patient the treatment best suited to their needs, the Block Center uses a series of diagnostic tests that identify a person’s “molecular fingerprint.” Since each patient’s biology is dynamic and continually changing, treatment options are continuously reviewed and altered based on each patient’s most current diagnostic information. Along with an individualized program of chemotherapy, Cancer Rehab is designed around this molecular fingerprint, with the goal of optimizing each patient’s fitness level so that they are up to the challenges chemotherapy often presents.

By structuring a program that includes the proper nutritional program and supplements to offset such side effects, and a fitness program that may include aerobics, weight training, meditation, Tai chi, yoga, or stretching exercises, plus the psychological support and counseling to get through treatment, many of these side effects can be minimized, or avoided altogether.
