A week’s vacation for the family on the warm, sunny beaches of Florida may seem like a great idea, until you find out that the only weeks available are smack in the middle of the spring school calendar, or that you need to pay double if you want to reserve dates during the holidays.

-- Don’t trust; verify. If you do your research and decide that the company offering a vacation package is on the up-and-up, go the extra mile for your peace of mind. Ask for the names and phone numbers of the airline and hotel involved, then call those numbers to confirm that your reservations have indeed been made, or ask them to confirm that they’re involved in travel packages from the company that contacted you.

-- Give your card issuer credit. The goal of any scammer is to get away with taking as much money from you as possible. Fortunately, federal law says that you’re only liable for the first $50 of any unauthorized charges to your credit card account, and many issuers won’t even charge you for that if you report the problem immediately. Therefore, your safest payment option for online purchases is usually your credit card, assuming you monitor your statements each month.

While some companies can afford to offer something for free as a way to increase brand awareness or store traffic, most “unbelievable” offers are unbelievable for one simple reason: They’re not true. Let caution and common sense be your travel guides as you navigate through scam offers.

National Private Duty Association Warns of Tax Implications with Household Help

The National Private Duty Association says that consumers who hire independent caregivers could be headed for tax problems if they don't understand their obligations.

Someone doesn't have to be running for a federal government cabinet post to be impacted negatively by the tax implications of hiring household help, according to the National Private Duty Association.

Nearly 25 percent of U. S. adults are caregivers for an older family member or friend, and many of these families are becoming accidental employers when they turn to a friend, acquaintance or individual to provide help for mom or dad.

The Internal Revenue Service (IRS) has clearly defined the household help category according to Jill Senso, education coordinator with the National Association of Tax Professionals, a nonprofit professional association dedicated to excellence in taxation.

"Household help is anyone who does work in or around your home," Senso said. "Examples would be babysitters, caretakers, housekeepers, gardeners, health aides, private nurses, nannies, yard workers or drivers. The worker becomes your employee if you control what work is done and how it is done."

Senso said the definition applies regardless if the person works full time or part time, was hired through an agency or list provided by an agency, or if you pay hourly, weekly, monthly or based on job completion.

According to the IRS, if consumers pay wages of $1,600 or more during the calendar year, they must withhold and pay social security and Medicare taxes. If consumers pay wages of $1,000 or more in any calendar quarter, they must pay federal unemployment taxes.

As home care choices escalate, an increasing amount of care is being provided by nurse registries and employment agencies that act as "matchmaker" services, assigning independent contractors to clients and patients in need of care. Many consumers who use these types of companies never realize that there are potential legal and tax consequences that go along with the use of an independent contractor.

"With so many providers of home care available, it can be very confusing, especially if help is being sought during an emotional time," said Kim Stoneking, executive director of the National Private Duty Association (NPDA). "Consumers, more and more, are turning to companies that offer them the cheapest price for care without understanding the implications of their choice. In many situations such as these, an employer/employee relationship is created between the consumer and the independent caregiver, and the consumer is unaware."

The National Private Duty Association is the nation's first association for providers of private duty home care, which includes non-medical home care services. NPDA members set themselves apart from other home care providers because of the supervision and training they provide, which assures a consumer is getting the best care possible.

"Private duty home care agencies are companies that provide home care aides, companion care, homemaker services and skilled nursing services in the patient's home," Stoneking said. "Additionally, members of the NPDA employ trained caregivers and assume all responsibility for the payroll and all related taxes - this is never passed off to the customer."

"I strongly urge people who find themselves in a position of needing household help to speak with a tax expert/accountant prior to doing so," Senso said. "The process is complicated and you would save yourself immensely by having a professional on board from the beginning. In many cases, when people have tried to handle payroll taxes on their own, mistakes are made. It proves to be more costly to amend after the fact, than to have them done correctly once the person has been hired."

Household Help Know-How

There are many unexpected or hidden issues when consumers hire independent caregivers. The National Private Duty Association offers these considerations when looking for a home care provider:

Payroll taxes: The simplest and most direct requirement is that anyone who gets a paycheck must pay the government any taxes due. This includes social security, Medicare, federal and state unemployment and state and federal payroll taxes. When the consumer is the employer and none of these taxes is being paid, there can be serious financial implications.

Work-related injuries: If no worker's compensation protection is provided (as mandated by law for employees in nearly every state), and the worker sustains an on-the-job accident, the liabilities can be substantial. Medical costs and disability payments could result in financial hardship for even a very wealthy consumer. Many believe their homeowner's insurance will cover such accidents; however, most policies exclude household help.

Caregiver supervision: Because of IRS regulations, registries and independent contractor agencies cannot provide any work supervision, scheduling or training to workers without becoming employers. For consumers and their families, hiring an NPDA member home care agency provides assurance that someone with experience and responsibility is reviewing the changing care needs of their loved one.

American Tinnitus Association Releases Hearing Conservation Tips for 2001

Portland, OR – You might instinctively throw your hands over your ears as a subway train screeches to a stop, but what about your hair dryer?

The two seemingly diverse noise generators actually register the same on a sound meter resonating at 100 decibels. At that level, the American Tinnitus Association (ATA) cautions that you can be well on your way to permanent hearing damage. It can result in something as aggravating as a constant ringing in the ears -- commonly referred to as tinnitus -- or as devastating as total hearing loss.

 Because such damage is largely preventable, the ATA is hoping the public will add hearing conservation to their roster of health pledges made in the New Year. To that end, the association has compiled a list of common environmental sounds and their decibel ratings to educate people about the relative risks for certain types of sounds.

A decibel is a unit for expressing the relative intensity of sounds with zero being the least perceptible and about 130 marking the average level of pain-inducing sound. Sounds of 85 decibels and above are considered dangerous. An MRI, for example, can save your life, but it also could damage your hearing if precautions are not taken. And those movie trailers we all enjoy may be costing us too as they are aired more loudly than the feature film.

You know the dangers, but what can be done about it? Earplugs offer an inexpensive and effective solution by cutting out about 15-20 decibels of loud sounds. If you are standing three feet away from someone and canÂ’t hear what that person is saying to you, the noise level is high enough to do damage.

Review labels on appliances and childrenÂ’s toys for noise levels. Encourage teens to take plugs to the amusement park or concerts. If a movie is too loud, take the initiative and ask that it be turned down. You will be doing everyone -- including the projectionist -- a service.

Tinnitus (pronounced ti-night'-us or tin'-i-tus) is the medical term for the perception of sound when no external sound is present. The scope of noise may include hissing, roaring, whistling, chirping or clicking and can be heard intermittently or constantly, in single or multiple tones.

Of the 50 million Americans affected by tinnitus, the effects range from subtle, even temporary, to a permanent life-altering level. It can strike people of all ages. At least 12 million among the known sufferers are so seriously afflicted that they cannot function on a "normal," day-to-day basis.

The ATA began in 1971 to provide research support. It grew to be recognized both nationally and internationally for its assertive stance on behalf of people with tinnitus.

Recent statistics from the Mount Nittany Medical Center in State College, Pennsylvania, indicate that the excessive drinking behavior often associated with Spring Break is not confined to the time students spend off campus, and instead persists throughout the year at alarming levels. Choose Responsibility (http://www. chooseresponsibility. org), an organization determined to address the binge drinking culture in America, recognizes the need to address this growing problem before it gets worse.

In 2008, emergency room personnel working near the campus of Penn State University treated a record number of students for alcohol related problems. These hospital visits were so common that the total number of students treated increased 84% since 2005. In addition, the average blood alcohol level of students seeking treatment rose from 0.234 to 0.252 percent, more than three times the legal limit for intoxication. 

“These are grim numbers,” said John McCardell, president of Choose Responsibility (http://www. chooseresponsibility. org). “Binge drinking is as serious an alcohol related problem today as drunk driving was two decades ago, and it is time for a serious solution. These statistics clearly show that dangerous, life-threatening drinking is not limited to one time of the academic year like Spring Break or fraternity pledge.” Despite the fact that high profile incidents often occur in Spring Break locations, America’s universities and communities are at the front lines of this problem throughout the year.

Across the country, alcohol contributes to some 1,700 deaths, 599,000 injuries, and 97,000 cases of sexual assault among college students every year. The U. S. Department of Health and Human Services says that approximately one in six teenagers have drunk so much that they blacked out and couldn’t remember what happened the night before.

“We need to teach our young people not to drink abusively by creating an alcohol culture that promotes responsible and healthy behavior,” said McCardell.

Thirteen years ago, when Amy Tucker was 19 years old, she faced a diagnosis of advanced cancer and the certainty of never having children, even if her cancer could be cured. Today, Tucker is cancer-free. She is also the mother of a beautiful baby boy, thanks to cancer fertility preservation (http://www. infertile. com/inthenew/lay/cancer-fertility. htm) procedures performed by Sherman Silber, MD, director of the Infertility Center of St. Louis at St. Luke's Hospital.

According to Dr. Silber, Tucker, now 32 and living in Columbia, Ill., is the first cancer survivor in the United States to give birth to a baby as a result of ovary freezing and transplantation.

"One in every 250 young women today is a cancer survivor, but their cancer treatment usually leaves these women sterile. In essence, it castrates these young girls chemically and radiologically while curing the cancer," said Dr. Silber. "With ovary freezing and transplantation, women can now preserve their ovaries and their fertility for the future after they are cured of their cancer. This medical advance brings a tremendous feeling of optimism to young women with cancer as well as to those who need to postpone childbearing for other reasons. It gives women with cancer the emotional message that we are expecting them to be cured, and 90 percent are cured."

In 1997, Tucker's treatment for Hodgkin's lymphoma began with six months of chemotherapy. She went into remission, but her cancer quickly returned. In 1998, Amy, then 20, needed more sterilizing cancer treatment. Fortunately, one of her healthcare providers told her about Dr. Silber's pioneering research in fertility preservation (http://www. infertile. com/infertility-treatments/preserving-your-fertility. htm), and she decided to have Dr. Silber remove and freeze one of her ovaries. After radiation, a bone marrow transplant and more chemotherapy, she was cured of her cancer and has been in remission since 2001. But the treatment that saved her life also left her menopausal and otherwise unable to have children.

In January 2009, Tucker returned to Dr. Silber so he could transplant her ovarian tissue that had been frozen more than a decade earlier. And on May 27, 2010, Tucker gave birth to a healthy baby boy, Grant Patrick Tucker.

Tucker, though the first cancer survivor in the U. S. to give birth as a result of these fertility preservation procedures, is one of a couple dozen women in the world who have chosen ovary freezing to protect them from the sterilizing effects of cancer treatment.
