More and more seniors are deciding to stay in their homes longer, with some planning to forego long-term care facilities entirely. This desire to continue living at home is changing the way seniors look at their future care, and it should be affecting their expectations of the coverage they receive from Medicare according to the Medicare Resource Center, a free online source of Medicare consumer information.

Each year, an increasing number of Baby Boomers become eligible for Medicare, and with that increase, more and more seniors are deciding to stay in their homes longer - with some planning to forego traditional long-term care facilities entirely.

A 2010 survey by AARP revealed that nearly three-quarters of respondents age 45 and older said they'd like to stay in their homes as long as possible.

This apparent desire is changing the way seniors look at their future care, and it should be affecting their expectations about the coverage they receive from Medicare, according to Chuck Smith-Dewey, founder of healthinsurance. org and its new Medicare Resource Center.

"Now, more than ever, seniors making plans to spend their golden years in their own homes should be thinking about how that decision will affect their quality of care and their personal finances," says Smith-Dewey. "Medicare covers only a fraction of in-home care expenses, but recipients should definitely seek advice about how the program can help with in-home expenses."

Seniors often have unrealistic expectations about Medicare, says Sheri Samotin, president of LifeBridge Solutions, a Florida firm that advises seniors and their families on how to plan for long-term care. "The system was never set up to pay for long-term care," she says. "My clients believe they have a wonderful Medicare card, and they're surprised when the majority of long-term care expenses aren't covered."

Some seniors and their caregivers mistakenly believe that Medicare will cover custodial care - paid services that assist seniors with activities of daily living such a bathing, preparing meals and transportation. "Medicare will never pay for true custodial care - at home or in a long-term care facility," Samotin says.

In truth, the vast majority of Medicare reimbursement for in-home care is for post-acute care such as physical, occupational or respiratory therapy. And that care is typically ordered by a doctor following inpatient treatment with a minimum stay. "In most situations, Medicare coverage of inpatient rehabilitation only follows inpatient admission or three days or more," Samotin says.

What in-home care can seniors expect from Medicare?

In-home physical therapy - In some cases, Medicare covers outpatient physical therapy at home. "If a patient has had a fall, or a doctor detects a change in functional status, the patient may qualify for physical therapy to improve their balance and endurance," says Nancy Hanley, Team Manager with MyHealthCareManager. com. That physical therapy may continue as long as the patient is progressing. "If the patient plateaus or is no longer benefiting from the service, the service is discontinued" she says.

Home health aides - Following acute care, a home health aide may also be covered under Medicare for a limited period of time if deemed necessary by the patient's physician. The services must, however, be related to the illness or injury treated an in conjunction with a skilled service, such as nursing, physical therapy, occupational therapy, respiratory therapy or speech therapy.

Home safety - Medicare can also help improve the in-home setting for Medicare recipients. The Medicare patient's physician can prescribe a home risk assessment by a physical or occupational therapist before the patient returns home from inpatient treatment.

"They'll check for clear pathways and make suggestions about rearranging furniture to accommodate wheelchairs and walkers," says Hanley. "They'll make recommendations for elevated toilet frames for stability, installing grab bars and even creating a downstairs bedroom in older homes when patients can't make it up the stairs."

Durable medical equipment - Physical and occupational therapists can secure durable medical equipment - hospital beds, bedside commodes, canes, walkers and wheelchairs - for the patient's use in the home with a doctor's order. In most cases, Medicare pays 80 percent of the cost of the prescribed equipment, but that equipment can only be prescribed once within a five-year period.

But Hanley notes that recipients should be aware that the covered equipment is limited. Medicare typically does not cover grab bars, shower stools and hand-held showerheads.

What about other long-term care expenses?

For the foreseeable future, seniors will have to look outside of Medicare for coverage of most long-term care expenses, Samotin say. "Medicare simply does not cover the vast majority of the help with day-to-day living that seniors often need. There are wonderful options if you can afford to purchase private companionship and care but they won't be paid for by Medicare."

Her advice? Think about long-term care insurance for coverage of in-home care. Depending on the policy, long-term care coverage could include skilled nursing care, physical therapy and assistance with activities of daily living (custodial care).

Also, check out the offerings of local social services organizations and - if you're a veteran - become familiar with VA benefits. But definitely don't wait.

"It's so important to people to buy long-term care insurance and to get in early," Samotin says. "If you buy in your 40s, 50s and 60s, your coverage may cost a few thousand dollars a year. If you wait until you're older or the point when you need it, the premiums are going to be much higher, or you may be uninsurable."

Organizations don't have it easy when deciding which open source firms to engage for implementation, migration, support, and other consulting services. To help organizations in the process, enterprise software analyst firm, Technology Evaluation Centers (TEC), launched its FOSS Evaluation Center (http://www. fossevaluation. com/prweb/ (http://www. fossevaluation. com/prweb/)) on February 4, 2005, which lets end users prioritize their requirements for open source migrations or open source consulting engagements, and then matches those requirements to appropriate open source consultants or service providers. The on-line tool lets end users analyze firms on approximately 1,000 prioritized criteria covering open source-related services. It also provides access to thousands of other detailed criteria for selecting open source-based enterprise systems and office migrations.

What should organizations seeking open source software do to prepare themselves before calling on an open source vendor or consulting firm for a request for proposal (RFP)? TEC's FOSS Evaluation Center (http://www. fossevaluation. com/prweb/ (http://www. fossevaluation. com/prweb/)) concentrates on complex software selection issues. According to TEC, solutions for ERP, CRM, or SCM may fail to accomplish what customers want, because customers did not begin their selection processes with a good understanding of their requirements. From TEC's vantage, the problem is that customers frequently do not prepare a proper request for information (RFI), and evaluate the pertinent responses from their shortlisted vendors. TEC believes its sophisticated means for performing requirements analysis will ease customers' selection burdens.

TEC has tracked significant increases in customer demand for Free and open source software selection projects over the last several years. For example, enterprises seeking solutions based on the Linux platform in Q3 2003 made up 9.5% of TEC's demand pool; however, that number increased to 10.8% in Q3 2004. Open source database platform demand went from 4% in Q3 2003 to 10.1% in Q3 2004. This signifies that the customers will need to rely on solid research for their software selection projects.

With an open source implementation or migration, more is involved than simply discerning the right software features. Open source solutions are apt to offer industry-specific versions from partners that have branched from the core open source project. For example, a "vanilla" open source ERP system might not be applicable in the health care industry but a partner firm working with the open source ERP project could create a specialized system meeting the exact needs of pharmacy management.

But how does the end user organization find out which partner firms offer the appropriate solution for its organization? TEC hopes to be this resource by providing its analysis and matching service to end users, consultants, and vendors.

Another frequently raised concern from organizations considering open source solutions is the availability of support. A lot of different factors must be taken into account to determine how well support services can be provided. This includes, for example, support components such as communication methods (how will the chosen firm make itself available?), processing time, self-help availability, local support presence, accountability, application or development support, etc. According to TEC analysts, every company or government organization that wishes to migrate to, or implement an open source solution, should thoroughly understand what it will need to request in terms of sponsored development, support, consulting, and training services from the firm it hires. And that is where things become a bit tricky.

For any given mature open source solution, organizations will find a number of firms claiming they can implement or provide support solutions for it. What is the customer organization to do? Which firm is most appropriate for its needs? To secure a successful project, it's got to figure out exactly what the organization requires from the firm it hires. TEC recommends prioritizing requirements to reflect what is most important, and knowing exactly how each of the company's shortlisted firms will support those requirements.

Blue Water Media combined elements from both Keller Crescent and Pharmagraphics in developing the new online identity of the combined forces. Some aspects of the new site were derived from the former Pharmagraphics website, which was also developed by Blue Water Media, and enjoyed an outstanding web presence. These aspects include the information infrastructure and some color influences. The primary colors of the new site were heavily influenced by Keller Crescent, as well as the logo that is being used to represent the joint operations. Blue Water Media implemented new navigational and page structures that give the site a modernized look, while also improving access to desired information. The combination of elements from both Keller Crescent and Pharmagraphics made for a seamless transition from both entities' older sites to the new web portal.

In addition to a new look and identity, Blue Water Media also implemented new backend functionality for the Keller Crescent website. Joomla!, an open source content management system, was implemented to allow for easy updates to any page of the website. This allows for individuals with little to no HTML or web design skills to be able to make content updates to their websites, which is a major advantage for Keller Crescent. Its staff can easily update product description pages to include additional features, or easily update details on office or manufacturing facility locations, products and services offered and more. This functionality will prove to be beneficial when needing to make immediate content updates to the website.

Scientists see troubling signs that we face growing risks for some health problems, and that these may be related to the human body burden of industrial chemicals.

In a study led by Mount Sinai School of Medicine in New York, in collaboration with the Environmental Working Group and Commonweal, researchers at two major laboratories found an average of 91 industrial compounds, pollutants, and other chemicals in the blood and urine of nine volunteers, with a total of 167 chemicals found in the group. Like most of us, the people tested do not work with chemicals on the job and do not live near an industrial facility.

Scientists refer to this contamination as a person’s body burden. Of the 167 chemicals found, 76 cause cancer in humans or animals, 94 are toxic to the brain and nervous system, and 79 cause birth defects or abnormal development. The dangers of exposure to these chemicals in combination has never been studied.

A field of 50 boats is expected to participate in the two-day tournament. Departing from the Vinoy Yacht Basin, they will fish in Tampa Bay tributaries for redfish, snook and trout. Each boat will carry a pair of anglers, along with a celebrity and professional fishing guide.

New this year will be an Offshore Angler Division presented by Treasure Island Charities, exclusively for fishermen who would like to explore the waters west of the beach. Beginning at 12:00 midnight on November 13, following The Fishermen's Ball, anglers in the Offshore Division will be eligible to leave their personal ports.

For the next 36 hours, offshore anglers may angle the Gulf of Mexico for the three species eligible in this division: amberjack, grouper and snapper.

Edgy Internet campaign focuses on workplace culture to empower professional women to breastfeed

Is the primitive fear of being caught naked causing countless women to quit breastfeeding each year?

That is one premise a California startup, Dao Health, is contributing to the public conversation about breastfeeding in an edgy new internet campaign that is taking a look at a variety of challenges different women face. “Our aim is to help women overcome various personal, social and structural obstacles to workplace lactation,” said Dan Garbez, cofounder of Dao Health. Other women fear that taking extra breaks to pump and requiring others to cover for them makes them look less dependable, less of a team player, or less committed to the success of their organization. So they don’t want to ask for any special accommodations.

Larger companies are required to provide private rooms and extra unpaid break time for women, if they request it, so these perceptions may not be a threat to every woman’s job. But many women nevertheless see pumping as a threat to their professional goals, especially in competitive environments.

Even if there is no downside for coworkers and the employer if a woman chooses to use a lactation room at work, these accommodations may very well be a threat to her desire to get home as early as possible to her family and baby, if pumping causes her to regularly get behind on her work. If she works in a small business, the choices may be even more difficult. If the small business can’t afford to accommodate her, her only practical choices may be to either give up working, or give up breastfeeding… That is, until now (http://freemie. com/freemieatwork. htm).

The entire story is gritty comedy, to satirize the point that many women are afraid of being seen naked by a coworker. But the not so subtle point is that mom can be a hero at work, and still bring home her milk.

The company is planning more edgy videos on more sensitive subjects. For example, many women don’t care what others think and will pump wherever they have to, but for others, their own self-image is the key impediment. “I can’t count how many women have told us they hated looking like a dairy cow. And these are just the women who laugh at themselves, and keep pumping anyway. But how many are so turned off by the vision in the mirror that it’s a brick wall for them?” asked Garbez.
